  3. Complete the Registration Form, Stamp and Sign it by your authorized signatories.
     
  4. Complete the Board Resolution, Stamp and Sign it by your owners.
     
  5. Submit the Form and the Supporting Documents to the nearest branch for your first-time enrollment:

Important Notes:

  • FATCA FORM: Non-American Business and Non-Financial Entities (Banks, Insurances companies) are categorized under D. Active Non-Financial Entity in FATCA Form.
     
  • The BisB Corporate Digital Form:
     
    • Usernames and Emails should be unique.
       
    • The authorized signatories should sign on all the pages in the form.
       
  • The Board Resolution: should be completed and printed on the company letterhead (if available).
     
  • The BisB Corporate Digital Form, FATCA and the Board Resolution should be submitted as hard copy.
     
  • If you wish to have more than 3 users, you could photocopy page 5 to add more users.
